All Entry-exit Personnel Shall Fill Out the Health Declaration Form

To better control the novel coronavirus-related pneumonia at points of entry and prevent its spreading across the border, in accordance with the Frontier Health and Quarantine Law of the People’s Republic of China, its implementation rules and other relevant laws and regulations, the General Administration of Customs of the People’s Republic of China (GACC) issued the Announcement on Re-adopting the Health Declaration Form for Inbound and Outbound Travellers on Jan. 26th. According tothe situation of epidemic prevention and control at points of entry and adjustments of relevant criteria, the fourth edition of the Exit/ Entry HealthDeclaration Form of the People’s Republic of China was officially put into use at 0:00 on March 3, 2020.

Filling out the health declaration form truthfully is to guarantee the life safety and physical health of the entry-exit personnel and others. True and accurate information enables the customs to keep up with the physical conditions of the travelers,make correct judgment accordingly, and take scientific and effective measures to control the spread of the epidemic cross-border, so that early detection, early report, early isolation and early treatment can be ensured. At the sametime, truthful declaration is also a legal obligation of the entry-exit personnel. If the entry-exit personnel conceal or falsely declare the information, resulting in the spread of the epidemic, he/she will be held accountable in accordance with the law.

Warm Reminder: All entry-exit personnel shall claim the paper version (see Figure 1 for details) from China Customs or at the airlinecounters or fill out the form online by scanning the QRcode of Customs Pocket Declaration (see Figure 2 for details) before entering/leaving China. All inbound and outbound travelers must present their electronic barcode orcompleted paper version of the Exit/ Entry Health Declaration Form of the People’s Republic of China to the officers of China Customs, and cooperate with temperature monitoring, medical examination and observation, as well as other public health measures. The travelers are free to pass after verification.

 

新型肺炎の防止抑制事業を徹底し、水際による拡散を防ぐために、『中華人民共和国国境衛生検疫法』及び関係細則に基づき、税関総署は1月26日に『出入国人員に向け健康申告カード記入制度の再起動について』を公表した。感染情勢と関係判断基準の変化により、『中華人民共和国出入国健康申告カード』第四版は2020年3月3日より利用開始となる。

事実通りに健康申告カードに記入することは出入国人員及び他人の生命安全と健康を守るためである。また、税関は申告の情報によって人員の健康状態をうまく把握する上、正しく判断し、科学的な措置を講じ、国境を越える感染を防ぐことができる。「早期発見、早期報告、早期隔離、早期治療」ができる。事実通りに申告することは出入国人員の義務であり、嘘な情報を提供したり、病状を隠したりすることで、新たな感染を引き起こす場合、法的責任を問われる。

注意事項:出入国人員全員は出入国前に税関や航空会社のカウンターで健康申告カード(図1)をもらうか、QRコードより「税関旅客指先サービス」アプリ(図2)で健康申告すべきである。出入国際、必ず税関の審査官に申告済みのバーコード或いは紙の『中華人民共和国出入国健康申告カード』を提供する他、体温観測や医学巡査、医学検問などを受け、審査官の許可を得てから通行できる。
